The prescription consult form is for current patients of Perfect Balance Equine Dentistry and Veterinary Services. Your animal/pet must have had an exam by a veterinarian with Perfect Balance in the last year to be eligible for a prescription consult. If your animal/pet has not been recently seen by a veterinarian at Perfect Balance, please schedule an exam by calling 480-389-5709. Requests will be reviewed on Monday-Friday during normal business hours. Please allow 48 hours for processing.
